ASSH 74 International Guest Lecture CRPS_02_201909

The meeting, with more than three thousand participants, took place at Caesars Palace in Las Vegas last week

Reflex sympathetic dystrophy, also known as Sudeck’s atrophy or CRPS1 (complex regional pain syndrome type 1) focused the main presentation of the Spanish surgeon, who doesn’t consider this atrophy profile as a pathology per se but as a diagnostic formula that masks diagnostic errors or the ignorance of a hidden condition. Thus, in Piñal’s opinion, many cases of chronic hand and wrist pain that “have a justification and, therefore, a solution” are mistakenly diagnosed as Sudeck’s atrophy.

The central argument of the lecture – titled “Reflections of a busy surgeon: reflex sympathetic dystrophy does not exist” – is based on a long investigation that includes the analysis of more than 100 cases of patients who came to Dr. Piñal with initial diagnosis of Sudeck’s atrophy, but in which he identified other pathologies that were addressed after the new diagnosis.

In addition to the complex regional pain syndrome, Dr. Piñal’s keynote speech in Las Vegas addressed the changes in upper limb surgery since the introduction of microsurgery in the late 1970s and arthroscopy in the first 2000s. Piñal also stressed the need to train a new generation of hand surgeons with a global perspective and not isolated subspecialists.

Together with the international guest conference, Dr. Piñal participated in several of the activities of the scientific-medical program, such as those dedicated to the management of carpal instability by arthroscopy or the state of the art in the therapeutic application of wrist arthroscopy.

Besides, the Spanish surgeon took part in the symposium ‘The ultimate thumb reconstruction’, in which the panelists analyzed the reconstruction surgery after traumatic loss of the thumb and the indications and results of its application in children with congenital differences.
